Purpose of collecting data
The lawful basis for Pro Resolve Therapy processing client data is:
- consent for personal data
- processing of special category personal data is necessary for the provision of healthcare
- to provide safe and effective massage therapy, information about client's current health is required alongside medical and injury history in order to provide best therapy and advice
- contact details are required for the arrangement of appointments in person and online
Where is the information gathered?
- Pro Resolve Therapy collects personal client information via the consultation form completed prior to receiving therapy
- Clients retain the right of refusal to supply personal identification information, however, this may prevent them from receiving treatment
- Pro Resolve Therapy also collects information by the recording of treatment notes following each treatment session
Uses of the information
- The consultation form is used as a way of assessing treatment requirements in order to provide an effective and safe treatment
- History of treatment record is used to identify trends and as a reference for any subsequent treatments
- Information given will never be shared, other than for required legal processes, without obtaining consent
- Medical information will never be shared unless in the event of an emergency whereby information may be passed to a third party such as the ambulance service or a medical professional
How long is information stored?
- Client data will be held for 5 years from the date of the client's last visit
- Records are required to be kept for insurance purposes
- Data will not be transferred without your consent
- Clients can, at any point, request to see or request copies of their own personal information
Client's Rights
The client possesses the following rights;
- Right to access personal data
- Right to limit the sharing of data
- Right to be forgotten and have data deleted (in circumstances that do not contravene our legal or insurance responsibilities)
- Right to be informed how we use personal data
Acceptance of Terms
- By using our services, you signify your acceptance of this policy
- Continued use of services following the posting of changes to this policy will be deemed an acceptance of such changes
